Step One:

Complete the Patient Registration PACKET to provide your sleep history, symptoms and demographic information to us.

Step Two:

We will verify you benefits (if applicable) and notify you about any out-of-pocket expenses you might incur.

Step Three:

Schedule an initial appointment with our Sleep Specialist to review your needs and determine the diagnostic path or test that is best for your conditions.  (We will request pre-authorization if required by your insurance.)

Step Four:

Schedule and complete the diagnostic test recommended by our Sleep Specialist.

 

Step Five:

Schedule a Results Appointment with our Sleep Specialist.

Prior to this meeting, our clinician/s will analyze all of the diagnostic results, review these results during your appointment and, if you wish, forward these results to your physician.
